To help patients-in-need, especially those with multiple medical conditions, SGH healthcare staff provide nursing services, doctor visits, tele-health consultations and medical support to reduce their risks of re-admissions. Working with partners to strengthen primary care, build capabilities and research in caring for the more vulnerable among us, we anchor care in the community so that patients can receive the best and most appropriate help in a familiar environment.
