We are Medical Social Workers from the inpatient team. We work closely with the multi-disciplinary team (MDT) and patients and/or their families to plan for the care and integration of our patients back to the community. We also provide financial support and supportive counselling to those in need.

We believe strongly in the quality of life and the continuity of care for our patients. We hope to continue to provide support to our patients through our various interventions and recommendations.

As Medical Social Workers, we are privileged to be at the front seat and be able to witness how the HomeCare Assist (HCA) Fund has benefited our patients and their family members who are in difficulty.
From emergency transport assistance to costly haemodialysis and savings-depleting consumables for patients with chronic illnesses, HCA allows this group of patients and many others to access medical care and continue to live their lives comfortably in the community.

Your generosity will help us continue to help our patients!
Support us as we come together to clock a total of 500km in support of the HomeCare Assist (HCA) Fund. We hope to raise $300 for HCA to provide interim assistance to needy patients with chronic illnesses or conditions to ensure that they have the means to continue to be cared for in their own homes after discharge from Changi General Hospital (CGH).
