This is Madam Tay Siet Huay who had benefitted from the care model under the Code 4 Home Care programme managed by Care for the Elderly Foundation (Singapore).

By 2030, Singapore is expected to have over 900,000 seniors aged 65 and over. With an increasing number of seniors, the demand for healthcare and aged care will grow. Ageing in place and at home remains the preference of the seniors.

Case of Madam Tay is one of the many that has been referred to The Foundation for home care service. We hope to be able to carry on this dedicated service and fulfill the wishes of the frail elderly to live and be as close to their love ones and families as long as they wish.

About the Charity

The Foundation was set up in 1995 as a society to provide home health care services that support ageing at home. It became a registered charity in January 1996 with Ministry of Social & Family Development as its Sector Administrator.

Its focus is now on the provision of home medical and home nursing services to the needy homebound elderly. As population of Singapore ages rapidly, more programmes and services are being planned.
